How to book an appointment
Appointments can be scheduled by contacting our clinic with a valid medical requisition.
Please bring your valid health card and the original requisition form to your appointment.

Referral and OHIP coverage
- You must be referred by a physician or other authorized health-care provider to be scheduled for an ultrasound exam.
- A written requisition form is required and must be signed by your doctor.
- Please bring your valid OHIP health card and your requisition form with you to the appointment.
Most diagnostic ultrasound tests are covered by OHIP when you present a valid health card and referral. If you do not have valid coverage, staff will inform you of any fees before the exam.
Same-day appointments are often available, especially when your exam does not require complex preparation. Call us to ask about current availability.
